    QUALIFICATIONS:

    Bachelor’s degree in pharmacy or Doctoral degree (PharmD) from an accredited institution. Active Pharmacist license in good standing from the state of Idaho, or in the process of obtaining one prior to start date.

    ADA ESSENTIAL FUNCTIONS:

    • Hearing: within normal limits with or without use of corrective hearing devices.
    • Vision: adequate to read 12-point type with or without use of corrective lenses.
    • Must be able to verbally interact with staff, clients and public.

    RESPONSIBILITIES:

    • Provides drug information, drug therapy consultation and staff education relating to drug therapy.
    • Ensures all prescriptions are processed and filled accurately and appropriately for each individual patient.
    • Participates in the development and provision of a full range of pharmaceutical care services to Marimn patients.
    • Consults and provides treatment plans to providers on the appropriateness of drug therapy on selected patients when drugs are the primary method of treatment.
    • Communicates directly with the Pharmacy Manager about problems that affect delivery of patient care.
    • Manages patients with complex cases effectively.
    • Treats patients and staff with courtesy and respect.
    • Manages therapy for selected patients based on pharmacy protocols.
    • Trusted by staff and patient for knowledge and opinions.
    • Demonstrates knowledge base in his/her field and keeps current through Continuing Education programs.
    • Functions as an interface between providers, patients, pharmacy and insurers on matters related to insurance verification and pre-certification/authorization.
    • Serves patients by providing friendly, courteous service and support in clinic and ensures a patient-centered approach to team-based care.
    • Rotates on the clinical floor, acting as a critical resource by providing pharmaceutical knowledge to the medical team and patients.
    • Works closely with our patients and providers to monitor the health of patients on anti-clotting drugs.
    • Provides education to patients on the challenges associated with anticoagulants and monitors blood levels while on the medication, adjusting dosage when necessary.
    • Develops and conducts drug utilization evaluations for the Center.
    • Adheres to formularies and rules governing the selection of medications.
    • Understands and possesses knowledge of drugs utilized for FDA-approved indications versus off-label indications.
    • Verifies that patients understand their medications and appropriate outcomes of their drug therapy.
    • Demonstrates timely and clear communication in regard to patient care in charting and personal communication.
    • Provides health promotion and disease prevention activities relating to drug use and preventative drug therapy to the community when needed.
    • Reviews and makes recommendations on the Pharmacy Policy and Procedures manual and other related documents necessary to meet AAAHC accreditation.
    • Participates in accreditation committees, attends staff, and Center meetings, and in-service training.
    • Properly documents errors in log and accepts responsibility for the number of errors that are appropriate for workload.
    • Supervises the delivery of pharmacy services and Pharmacy Technicians.
    • Handles administrative problems and patient complaints when needed.
    • Requisitions and maintains adequate supplies for the department.
    • Assures availability, preparation, and control of medications.
    • Assists in identifying problems and helps with constructive problem solving
    • Communicates directly with the Pharmacy Manager about problems that affect the pharmacy operations.
    • Promotes a cooperative team effort within the pharmacy
    • Consults with the billing office staff in third party processing of pharmacy billing as well as participate in the development of an integrated billing software package.
    • Contributes equally and positively to the workload in the pharmacy.
    • Makes productive use of administrative and down time.
    • Develops programs and ideas to improve efficiency of the department.
    • Accesses computers for appropriate data entry, pharmacy files, patient charts, inventory, etc.
    • Manages personal and professional priorities for the good of the clinic.
    • Demonstrates compassion and understanding of patient’s problems.
    • Works to effective resolve problems:
      • 5- NO complaints
      • 4 - <2 complaints/year
      • 3- (3-5 complaints/year)
      • 1- >5 complaints/year
    • Consistently works as a team player in completing pharmacy workload and facilitating patient care.
    • Maintains Pharmacy logs – daily log and controlled substances.
    • Participates in the Tobacco Cessation Clinic when necessary.
    • Other duties as assigned
